RESUMO

OBJECTIVE: Assess risk factors, local and systemic immunological biomarkers in healthy individuals and with Denture Stomatitis (DS). DESIGN: For this observational transversal study, 27 participants without DS (Group 0), 24 with moderate DS (Group 1), and 25 with severe DS (Group 2) were assessed for sociodemographic, behavioral, and clinical parameters, microbial load of Candida spp., Staphylococcus spp., Streptococcus mutans, Pseudomonas spp., and enterobacteria, and cytokine and C-reactive protein levels. ANOVA, Fisher's exact, Kruskal-Wallis, Mann-Whitney, Wilcoxon and Pearson's chi-square tests were used for data analysis (α = 0.05). RESULTS: Group 1 had a significantly higher mean age compared to the other groups (P = 0.018), but no correlation was identified between age and DS (P = 0.830; r = 0.025). No significant differences were found among the groups for other sociodemographic and behavioral characteristics. Group 1 had significantly older upper and lower dentures; however, no correlation was identified between age of upper (P = 0.522; r = 0.075) and lower (P = 0.143; r = 0.195) dentures and DS. The microbial load of Candida albicans on the dentures (P = 0.035) and Candida spp. on the palate (P = 0.008) of the groups 1 and 2 was higher than group 0. Group 1 and 2 had higher Candida spp. counts on denture (P = 0.003) than group 0. There was no difference among groups for bacterial analyzed. Group 1 showed higher and Group 2 intermediate salivary levels of IL-6 compared to Group 0. There was no difference in the C-reactive protein levels among groups. CONCLUSIONS: Microbial load of Candida spp. is the factor with the strongest relationship with DS, with capacity for local signaling through IL-6.

STATEMENT OF PROBLEM: Denture-related stomatitis (DRS), an inflammation frequently present in human immunodeficiency virus-positive (HIV+) individuals, can be attributable to colonization by Candida spp., which is considered a main factor. The virulence factors of these species are often modulated by the systemic condition of their hosts. PURPOSE: The purpose of this clinical study was to evaluate the incidence, virulence, and morphology of Candida spp. isolated from biofilms of complete denture wearers with DRS, with and without an HIV diagnosis. In addition, the interaction of the systemic condition with the ability of Candida spp. to colonize was evaluated. MATERIAL AND METHODS: Fifty-five complete denture wearers diagnosed with DRS were divided into 2 groups: experimental (HIV+) and control (human immunodeficiency virus-noninfected participants [HIV-]). Biofilm was collected by a standardized method of ultrasonification of prostheses. The incidence was evaluated by a chromogenic method and polymerase chain reaction (PCR). The virulence factors were assessed by using the capacity for biofilm formation by counting colony-forming units (CFUs/mL), biofilm metabolism by tetrazolium salt metabolization, and proteinase and phospholipase production by using a fluorimetric kit. Morphology was verified by using the hyphae-inducing test, and participants' health data were collected with a form. Data were analyzed by using the Student t, Mann-Whitney U, Spearman, and Fisher tests (α=.05). RESULTS: The results of incidence were related to 55 participants (22 experimental and 33 control); in total, 63 Candida spp. samples were isolated, showing 28 Candida albicans and 36 nonalbicans strains. No significant difference was found between groups in baseline CFU/mL counts, biofilm formation capacity, cell metabolism, and phospholipase production. Proteinase production was higher for C. albicans in the control (P=.031) and for nonalbicans in the experimental (P=.016) groups. Relative to health data, the experimental group showed a moderate negative correlation between the CFU count/mL at baseline for nonalbicans and DRS classification (P=.020). CONCLUSIONS: C. albicans was the most prevalent species. No difference was found in the Candida spp. of complete denture wearers with DRS, with and without an HIV diagnosis, with regard to virulence factors (except for proteinase production) and morphology.

STATEMENT OF PROBLEM: Denture stomatitis affects complete denture wearers and is frequently treated with antifungals drugs, as well as treating the denture with sodium hypochlorite. Whether the limitations of these treatments can be overcome with local hygiene protocols that do not damage the denture materials or adversely affect the patient is unclear. PURPOSE: The purpose of this randomized controlled trial was to evaluate the effect of denture hygiene protocols on complete denture wearers with denture stomatitis. MATERIAL AND METHODS: For this randomized, double-blind controlled clinical trial, 108 participants were assigned to parallel groups: 0.25% sodium hypochlorite (positive control) 0.15% Triclosan, denture cleaning tablets, or denture cleaning tablets plus gingival cleaning tablets. The participants were instructed to brush the dentures and the palate and immerse the denture in the solutions. The outcomes of denture stomatitis remission, biofilm removal, decrease of microbial load (colony-forming units), and odor level of the mouth and denture were measured at baseline and after 10 days. Descriptive analyses were used for sociodemographic characterization of the participants; the Pearson chi-square test was used to compare participant frequency with different degrees of denture stomatitis. The data were not normally distributed (Shapiro-Wilks test) or homogeneous (Levene test). So, the Kruskal-Wallis and Dunn post hoc tests and Wilcoxon test were used to compare the effects of solutions and time on the variables (α=.05). RESULTS: The frequency of the highest to lowest denture stomatitis scores was significantly different for the 0.15% Triclosan and denture cleaning tablets groups. No significant difference was found among the groups in terms of denture stomatitis scores, biofilm, or colony-forming unit count of Candida spp. or C. albicans and S. mutans; a significant reduction was found in these parameters. The 0.25% sodium hypochlorite and 0.15% Triclosan treatments caused a significant reduction in Gram-negative microorganisms; these 2 protocols, and the denture cleaning tablets showed a significant reduction in Staphylococcus spp.; all protocols had similar effects. Only the S. mutans count of the palate decreased after 10 days. The odor level of the mouth and the denture was not significantly different (P=.778). CONCLUSIONS: The evaluated protocols can be recommended for the hygiene of complete dentures, since they were effective for all the variables studied.

PURPOSE: To evaluate the effects of 0.2% sodium hypochlorite, Efferdent (Prestige Consumer Healthcare), and 6.25% Ricinus communis on biofilm removal and antimicrobial action on dentures and brushes using nonimmersion or immersion protocols for the brushes. MATERIALS AND METHODS: A total of 45 denture wearers were randomly assigned to a denture immersion protocol for 7 days: 0.85% saline solution for 20 minutes (control); 0.2% sodium hypochlorite for 20 minutes (SH); Efferdent for 3 minutes; or 6.25% Ricinus communis for 20 minutes (RC). The participants were also randomized to immersion (n = 23) or no immersion (n = 22) of their brushes with their dentures in the same solutions. For biofilm evaluation, the dentures were stained and photographed, and the area of the biofilm was measured using Image Tool 3.0 (University of Texas Health Science Center). To evaluate microbial load on dentures and brushes, the biofilm was collected, and the Candida spp and Streptococcus mutans colonies were counted. RESULTS: The SH, Efferdent, and RC groups showed reduced biofilm and Candida spp on dentures regardless of the immersion protocol for the brushes. However, no difference was found in the Candida spp counts collected from the brushes immersed compared to the brushes not immersed in the solutions. The SH and Efferdent groups showed reduced S mutans on both dentures and brushes, except for in the nonimmersion subgroups. CONCLUSION: All solutions reduced denture biofilm and microbial load. However, immersion of brushes in the solutions did not contribute to reducing the microbial load.

The aim of this qualitative study was to gain a deeper understanding of patient perceptions of wearing implant-retained overdentures with ball-shaped or cylindrical attachment systems. Twenty-two wearers of implant-supported overdentures participated in this qualitative study based on a randomized crossover clinical trial that aimed to compare a cylindrical attachment and a ball attachment. In phase I of the study, group A experienced ball attachments (n = 11) and group B Locator attachments (n = 11) for 1 year. Afterward, in phase II, the attachments were changed; group A received Locator attachments and group B received ball attachments. One week after the attachment's replacement, semistructured individual interviews were conducted. All interviews were audiotaped and transcribed. The analysis was guided by thematic content analysis. Most of the patients from both groups preferred the attachment they received in phase II, regardless the type. A major theme raised by the participants to justify their preference between the attachment types was prosthesis retention/stability, sometimes considered as a positive and other times as a negative factor. Other themes were also explored: oral function, pain, hygiene, previous experiences, confidence on the dentist's work, and esthetic. Aspects related to the retention/stability of the overdentures are the main concerns associated with the perceptions of most patients treated with implant overdentures regardless of the type of attachment. Adequate retention level should be identified and adjusted on an individual basis and maintained overtime as possible. Therefore, follow-up appointments should be planned for readjustment of the attachment's retention. Overretention should be avoided.

PURPOSE: To evaluate the in vitro antimicrobial efficacy of alkaline peroxides against microbial biofilms on acrylic resin surfaces. METHODS: Denture base acrylic resin (Lucitone 550; n= 360) circular specimens (15 x 3 mm) were obtained from a circular metal matrix and sterilized with microwave irradiation (650 W, 6 minutes). The specimens were then contaminated with suspensions [106 colony-forming units (CFU)/mL] of Candida albicans (Ca), Candida glabrata (Cg), Staphylococcus aureus (Sa), Streptococcus mutans (Sm), Bacillus subtilis (Bs), Enterococcus faecalis (Ef), Escherichia coli (Ec), and Pseudomonas aeruginosa (Pa). After contamination, the specimens were incubated at 37 degrees C for 48 hours and then placed in a stainless steel basket, which was immersed in a beaker with one of the following solutions prepared and used according to the manufacturers' instructions (n= 10 per group): Group PC (positive control), phosphate-buffered saline (PBS) solution; Group MI, NitrAdine, Medical Interporous; Group EF, Efferdent Plus; Group CT, Corega Tabs; and Group NC (negative control; n= 5), no contamination and immersed in PBS. After incubation (37 degrees C, 24 hours), the number of colonies with characteristic morphology was counted, and CFU/mL values were calculated. The data were processed following the transformation into the formula log" (CFU + 1) and statistically analyzed by the Kruskal-Wallis and Dunn's tests (alpha = 0.05). RESULTS: There were significant differences between the groups for the evaluated microorganisms with a significant reduction in the CFU/mL. MI was effective for Ca, Cg, Sa, Sm, Ef, Ec and Pa; EF was effective for Cg, Sm, Ef, Ec and Pa; and CT was effective for Sa, Bs and Ec, when compared with the PC group.

OBJECTIVE: This study evaluated colour stability, hardness and roughness of soft denture liners after immersion in various cleansers. MATERIALS AND METHODS: Thirty specimens (14 mm × 4 mm) of Elite Soft Relining (ES) and Mucopren Soft (MS) were randomly immersed in distilled water at 37°C, sodium hypochlorite 1%, and an experimental Ricinus communis solution (RC) for 7, 15 and 183 continuous days. RESULTS: anova (p < 0.05) and Tukey's test indicated that after T7 (µ =8.79 ± 7.36); T15 (µ = 4.23 ± 2.62) and T183 (µ = 8.78 ± 3.16), MS presented a higher increase in hardness than ES. After T7, MS underwent an increase in roughness (µ = 0.09 ± 0.80); ES underwent a decrease (µ = -0.08 ± 0.16). RC caused the smallest variation in roughness. After T15, both materials presented an increase in roughness. After T183, ES (µ = -0.30 ± 0.48) presented a higher roughness variation than MS (µ = -0.07 ± 0.32). Hypochlorite caused an increase in roughness (µ = 0.02 ± 0.19). CONCLUSION: After all periods ES presented higher colour alteration than MS; highest colour alteration was caused by hypochlorite. Both materials were more stable after immersion in RC.

OBJECTIVE: To evaluate 16 patients of both sexes with lower overdenture and upper complete dentures, by analysing the resonance frequency of the initial and late stability of implants used to retain the overdenture under immediate loading. BACKGROUND: Oral rehabilitation treatment with complete dentures using implants has been increasingly more common among the specialists in the oral rehabilitation area. This is an alternative for obtaining retention and stability in treatments involving conventional complete dentures, where two implants are enough to retain the overdenture satisfactorily. MATERIALS AND METHODS: The Osstell™ Mentor device was used for the analysis in the initial period (primary stability), 3 and 15 months after the installation of the lower overdenture (secondary stability). The statistical analysis was performed with the repeated measures model (p < 0.01). RESULTS: The implant stability quotients were observed to increase after 15 months of the rehabilitating treatment. CONCLUSION: The use of overdentures over two lower implants should become the treatment of choice for individuals who have a fully edentulous mandible.

OBJECTIVE: To evaluate the antimicrobial action of effervescent tablets and ultrasound on Candida spp. and mutans streptococci from denture biofilm. BACKGROUND: It is not uncommon for edentulous patients to be elderly and find it difficult to brush their dentures. Hence, auxiliary methods are required for cleansing dentures as well as treating oral infections. MATERIALS AND METHODS: Seventy-seven complete denture wearers were randomly assigned into four groups: (A) Brushing with water (control); (B) Effervescent tablets; (C) Ultrasonic device (Ultrasonic Cleaner, model 2840 D); (D) Effervescent tablets and ultrasonic device. All groups brushed their dentures with a specific brush and water, three times a day, before applying their treatments. Denture biofilm was collected at baseline and after 21 days. The samples were collected by brushing the dentures with saline and the detached microbial cells were quantified by plating. Counts [log (CFU+1) ml(-1) ] of total aerobes, Candida spp. and mutans streptococci were compared by one-way anova or Kruskal-Wallis test (α = 0.05). RESULTS: No significant difference was found among the methods from C. albicans (p = 0.76), C. tropicalis (p = 0.94) and C. glabrata (p = 0.80). Lower counts were found for methods B and D when compared with the other methods against mutans streptococci (p < 0.001). Method B showed lower total aerobic counts than A, whereas C and D showed intermediate results (p = 0.011). CONCLUSION: The effervescent tablets significantly reduced mutans streptococci and total aerobes from denture biofilm. However, they was not as effective against C. albicans. Ultrasonic cleansing presented a discrete antimicrobial effect and was less effective than the tablets for complete denture disinfection.

INTRODUCTION: Millions of people around the world do not have access to the benefits of osseointegration. Treatments involving oral rehabilitation with overdentures have been widely used by specialists in the oral medicine field. This is an alternative therapy for retention and stability achievement in total prosthesis with conventional treatment, and two implants are enough to establish a satisfactory overdenture. OBJECTIVE: The objectives of the study were to evaluate 16 patients of both sexes, with an average age of 47.4±4 years, using electromyographic analysis of masseter and temporal muscles and analyse the increase of incisive and molar maximal bite force with their existing complete dentures and following mandibular implant overdenture therapy to assess the benefits of this treatment. MATERIALS AND METHODS: For these tests, the Myosystem-BR1 electromyograph and the IDDK Kratos dynamometer were used. Statistical analysis was performed using the repeated measures test (SPSS 17.0). RESULTS: A decrease in electromyographic activity during the rest, lateral and protrusion movements and increase of the maximal incisive and molar bite force after 15months with a mandibular implant overdenture was observed. CONCLUSION: All the patients in this study reported a considerable improvement in the masticatory function and prostheses stability following treatment. It is possible to propose that the use of mandibular implants overdenture should become the selected treatment for totally edentulous patients to facilitate oral function and quality of life.

OBJECTIVES: This study evaluated colour stability, surface roughness and flexural strength of microwave-polymerised acrylic resin after overnight immersion in sodium hypochlorite, simulating 180 days use. MATERIALS AND METHODS: Forty disc-shaped (15 mm x 4 mm) and 40 rectangular specimens (65 mm x 10 mm x 3 mm) were prepared from microwave-polymerised acrylic resin. The specimens were immersed in 0.5, 1% sodium hypochlorite, Clorox/Calgon and distilled water. Colour measurements (ΔE) were determined by a portable colorimeter. A surface analyser was used to measure roughness (µm). The flexural strength (MPa) was measured using a three-point bending test in a universal testing machine. Data were evaluated by one-way anova, followed by Student-Newman-Keuls test (α = 0.05). RESULTS: Statistical analysis found significantly higher colour changes (SNK, p < 0.001) for the 1% sodium hypochlorite, but mean ΔE value quantified by National Bureau of Standards was classified as slight. When comparing the surface roughness, no statistical significance was found among the solutions (anova, p = 0.637). The 1% sodium hypochlorite presented significantly lower flexural strength compared with the control group (SNK, p = 0.034). CONCLUSION: It was concluded that immersion in 1% sodium hypochlorite solutions for 8 h does influence the colour stability and flexural strength of microwave-polymerised acrylic resin, during the simulated period of 180 days.

OBJECTIVES: The purpose of this study was to evaluate biofilm distribution over internal and external surfaces of upper complete dentures. It was also aimed at assessing the effect of oral hygiene instructions before and after home use of a disclosing solution. MATERIALS AND METHODS: The sample consisted of 29 complete denture wearers whose upper prostheses were evaluated. Surface biofilm was disclosed by means of a 1% neutral red solution and quantified with regard to internal and external surfaces. Oral hygiene information was provided on an individual basis. During the second stage, an amount of disclosing solution was given for domestic use. RESULTS: Internal and external surfaces presented a similar amount of biofilm, which was concentrated over the area of the posterior teeth area, palatal rugae and the internal vestibular incline of the distobuccal flange. This pattern was maintained during the study. However, overall amounts were reduced following denture hygiene information. The use of disclosing solution by subjects at home resulted in a further reduction. CONCLUSIONS: It was concluded that biofilm deposits were similar over assessed surfaces, regardless of the type of procedure but irregular areas presented greater amounts. Oral health instruction was effective in reducing the biofilm, in particular when associated with home use of a disclosing agent.

OBJECTIVE: This report evaluated the efficacy of three brushes and one biofilm disclosing agent in complete denture cleansing. METHODS: Twenty-seven wearers of maxillary dentures were distributed into three groups and received different brushes: Oral B40, conventional toothbrush (Oral B); Denture, denture-specific brush (Condor); Johnson & Johnson, denture-specific brush (Johnson & Johnson). The 60-day experimental period was divided into two techniques: I - brushing (brush associated with a paste - Dentu Creme, Dentco) three times a day; II - brushing and daily application of 1% neutral red on the denture internal surface. Biofilm quantification was carried out weekly and the areas with dye biofilm were obtained by means of Image Tool 2.02 software. RESULTS: Biofilm removal was more effective during Technique II (Wilcoxon test: p=0.01) for the three groups of brushes. When the brushes were compared in Technique I, the Kruskal Wallis test indicated statistical difference between Denture X Johnson & Johnson and Denture X Oral B40, in which the Denture was more efficient. For Technique II, there was no statistical difference between brushes (p>0.05). CONCLUSION: The disclosed application promoted more efficacy on biofilm removal, regardless of the brush used. Denture (Condor) was more efficient than the other brushes during Technique I.

This report describes the use of a removable acrylic resin tray handle that can be easily attached to custom impression trays to produce an improved peripheral sealing zone. This device can be indicated to develop functional impressions for complete dentures using the patient-conducted muscular motion technique. In upper trays, the handle is fixed in the midline with acrylic resin, while in lower trays the centrally positioned handle is removed before border molding. This removable handle allows patient's suction and free tongue movements. Final impression is carried out in two stages: peripheral sealing (low fusion compound) and recording of the main supporting region of the denture (zinc oxide and eugenol paste). All border records are obtained from the patient's own movements (handle suction and tongue motion). The removable handle is simple to use, is reusable, can be adapted to any individual acrylic resin trays and allows accurate registration of the peripheral sealing zone (border tissues).

This study compared the application and reliability of 4 methods for biofilm quantification (computerized, paper-weighing, point-counting, and planimetric) in complete dentures, verifying the correlation between them. The internal surfaces of 62 complete dentures were stained (5% erythrosine) and photographed. The slides were projected on paper, and the areas (total and biofilm-covered) were outlined with a pencil. These areas were measured with an equidistant point grid (point-counting method), a digital planimeter (planimetric method), and for the paper weighing method they were cut and weighed with a precision scale. For the computerized method, ImageTool software was used. In order to perform a validation test of the methods, all of them were applied to slide projections of geometric figures with known dimensions. The correlation tests showed high correlation values (r = 0.82 to 0.99) among the methods. The validation test (ANOVA) showed no statistically significant differences among the values obtained from the measurement of figures using all four quantitative methods and the real dimensions of these geometric figures. Quantitative methods were efficient and reliable for measuring quantity of biofilm in complete dentures, and may be useful in experimental studies on the efficacy of hygiene products. The computerized method was fast and easy to perform.
